Assessment Process and Completion of the Questionnaire


When we receive a completed Care Home Payments UK Continuing Healthcare Questionnaire we will provide a free review. We will assess the information provided against the NHS Criteria for NHS Continuing Healthcare, by assessing the twelve health domains used by the NHS, the four key indicators and the incidental/ancillary test.


If we feel, based on the information provided in the Care Home Payments UK Continuing Healthcare Questionnaire, that the patient would not meet the criteria for NHS Continuing Healthcare you are entitled, and within your rights, to seek further guidance, or request an assessment direct with the NHS.
